This information is provided to students, spouses, and parents to explain what is required when the various items listed below are requested by Arkansas State University – Mountain Home Office of Scholarships and Financial Aid.

Students that have filed the 2014-2015 Free Application for Federal Student Aid (FAFSA) and have been admitted to Arkansas State University – Mountain Home will receive emails requesting additional documents or actions depending upon how the 2014-2015 FAFSA was filed. This request for additional documentation or action will only occur if a student has met both of the following criteria:

  • Admitted to Arkansas State University – Mountain Home
  • Filed the 2014-2015 Free Application for Federal Student Aid (FAFSA) at https://fafsa.ed.gov/ and used the school code of 001090

Emails requesting additional information will be sent to the email address provided on the 2014-2015 FAFSA. If no valid email address was provided, letters will be mailed to the permanent address provided on the 2014-2015 FAFSA.

Until ALL requested documents have been provided and/or requested action has been taken, no offer of federal financial aid will be made and emails will continuously be sent.

Please allow 10 days to two weeks for our system and the email or letter communication to be updated after the requested item(s) are submitted.

  • The student either left FAFSA question 23 blank or reported that having a conviction for possessing or selling illegal drugs which may make the student ineligible for federal financial aid.   • The student’s file is being reviewed and we are in need of additional information to determine the student’s eligibility because an unusually low income was reported.

2014-2015 Marital Status Verification Form - Top of Page

  • Complete, sign, and submit the 2014-2015 Marital Status Verification Form, along with the required documentation as indicated, to the Arkansas State University – Mountain Home Office of Scholarships and Financial Aid.
  • A possible conflict between a marital status and the federal tax return filing status reported on the FAFSA must be resolved before the student's Federal Financial Aid eligibility can be determined.

2014-2015 Statement of Educational Purpose - Top of Page

  • In the presence of a notary public or an official from the Arkansas State University – Mountain Home Office of Scholarships and Financial Aid, sign, complete and submit the 2014-2015 Statement of Educational Purpose form to the Arkansas State University – Mountain Home Office of Scholarships and Financial Aid.
  • Provide a copy of a valid government-issued photo identification (ID), such as a driver’s license, other state-issued ID, or U.S. passport. The ID card cannot be expired.
  • the 2014-2015 Statement of Educational Purpose form cannot be faxed and must be notarized by a notary public unless it is signed in the presence of or an official from the Arkansas State University – Mountain Home Office of Scholarships and Financial Aid.

2014-2015 Verification of Dependency Status Form - Top of Page

  • Complete, sign, provide the requested additional information, and submit the 2014-2015 Verification of Dependency Status form to the Arkansas State University – Mountain Home Office of Scholarships and Financial Aid.
  • The student’s file is being reviewed and we are in need of additional information to determine if the dependency status the student indicated on the FAFSA is correct.
  • The 2014-2015 Verification of Dependency Status form indicates what additional information the student must provide to the Arkansas State University – Mountain Home Office of Scholarships and Financial Aid to confirm dependency status.

Aid Suspended - Student Appeal Required - Top of Page

  • The student is considered ineligible for Federal Financial Aid because student is not maintaining the required Satisfactory Academic Progress.
  • Student must submit a letter of appeal to the Arkansas State University – Mountain Home Office of Scholarships and Financial Aid.
  • Documentation that substantiates statements in a letter of appeal are recommended, but not always required.  The appeals committee may request documentation that substantiates statements made prior to considering an appeal.

    • Provide a copy of member copy 4 of the DD214 Form for the student’s to the Arkansas State University – Mountain Home Office of Scholarships and Financial Aid.
    • The student indicated a status as a veteran on the FAFSA did not match the Veterans Administration records. The student’s copy 4 of the DD214 Form is being requested in an effort to resolve the mismatch.

    • Our records indicate that you are in default.
    • A Letter is needed from the lender/servicer or guarantee agency holding the defaulted loan(s) which provides clearance for each loan in default.
    • Students can view the status of their outstanding Federal Student Loan(s) by logging into the National Student Loan Data System at http://nslds.ed.gov/.

    Document Resolution of Previous Federal Aid Overpayment - Top of Page

    • Our records indicate that you have an unresolved over-payment of Federal Aid from a previously attended educational institution.
    • A Letter is needed from the previously attended educational institution reflecting you no longer owe an over-payment.  
    • Students can view their Federal Student Aid status by logging into the National Student Loan Data System at http://nslds.ed.gov/.

    Exceeds Loan Limits - Top of Page

    • Our records indicate you have reached or exceeded the allowable limits for Federal Direct Subsidized and/or Unsubsidized Loans. 
    • Your application for Federal Financial Aid has been suspended because you may be ineligible for Federal Financial Aid. 
    • Contact the Arkansas State University – Mountain Home Office of Scholarships and Financial Aid for further assistance.

    Letter Requesting Additional Information - Top of Page

    • You application for Federal Financial Aid has been suspended because the Arkansas State University – Mountain Home Office of Scholarships and Financial Aid has sent a letter requesting additional information.
    • Provide the additional information requested as soon as you received the letter or contact the Arkansas State University – Mountain Home Office of Scholarships and Financial Aid for further clarification.

    Loans Dischared because of Disability - Top of Page

    • Persons granted a Total and Permanent Disability (TPD) Discharge on federal student loans or TEACH Grant service obligations, are not eligible to receive a new loans in the future unless:
      • You obtain a certification from a physician that you are able to engage in substantial gainful activity; and
      • You sign a statement acknowledging that the new loan obligation cannot be discharged in the future on the basis of any injury or illness present at the time the new loan is made, unless your condition substantially deteriorates so that you are again totally and permanently disabled.
      • In addition, if your discharge was granted based on documentation from the SSA or a physician’s certification and you request a new Direct Loan, or Perkins Loan during the 3‑year post‑discharge monitoring period, you must resume repayment on the previously discharged loans or acknowledge that you are once again subject to the terms of your TEACH Grant service obligation before you can receive the new loan.
    • The proper paperwork is available from your current loan servicer.  Log onto the National Student Loan Data System at https://www.nslds.ed.gov/ using your Federal Student Aid PIN to determine the current holder(s) of your loans.

    Online - Direct Loan Entrance Counseling - Top of Page

    • Our records indicate that you have been offered a Direct Stafford Loan.  One of the requirements which must be met to receive a Direct Stafford Loan is Online Direct Loan Entrance Counseling available at www.studentloans.gov.

    Parent Signature of Federal Signature Page - Top of Page

    • The dependent student’s FAFSA was not signed by a parent.  A parent signature is required on the Free Application for Federal Student Aid (FAFSA). 
    • In order to submit or make corrections and sign the FAFSA online at https://fafsa.ed.gov/, both the student and a parent must sign using their own separate Federal PIN number.
    • A Federal PIN number can be requested at http://www.pin.ed.gov.
    • Federal PIN numbers should be stored in a safe place for future use.
    • Allow 10 days to two weeks for the student’s status to update in our system after using the Federal PIN number to sign the FAFSA. 
    • Each update to a FAFSA can result in a request for additional information.

    Parent's 2013 Tax Information - Top of Page

    • A student chosen for verification whose parent(s) filed or will file a 2013 Federal Income Tax Return and has not already used the IRS Data Retrieval Tool must use the IRS Data Retrieval Tool, if possible, or provide IRS Tax Return Transcript(s) as part of the requested Verification documentation.
    • If possible, log into the student's FAFSA at https://fafsa.ed.gov/ and complete the IRS Data Retrieval Tool in the Parent's Financial Information.  Remember, both the student and parent must sign the FAFSA with their Federal PINs each time a correction is submitted.     • If using the IRS Data Retrieval Tool to load parental Federal Income Tax information is not possible, provide a copy of the parent(s) IRS Tax Return Transcript(s) to the Arkansas State University - Mountain Home Office of Scholarships and Financial Aid.  You must set up an account with the IRS and log in to be able to download IRS Tax Return Transcript(s).  
    • If neither the IRS Data Retrieval Tool or IRS Tax Return Transcript(s) are possible review the requested Verification Form for additional assistance before contacting the Arkansas State University - Mountain Home Office of Scholarships and Financial Aid.
    • All changes made to the FAFSA online require the use of a Federal PIN number available at http://www.pin.ed.gov. Making changes to a dependent student’s FAFSA online requires both the student and parent to use separate Federal PIN numbers. Federal PIN numbers should be stored in a safe place for future use.
    • Allow 10 days to two weeks for corrections made to the student’s FAFSA to update in our system.
    • Each update to a FAFSA can result in a request for additional information.

    Pell Lifetime Eligibility - Top of Page

    • There is a limit to the total amount of Federal Pell Grants that a student may receive.  Your records indicate that you are either close or have reached the maximum limit.  Your Pell eligibility may be limited.

    Proof of High School Completion Status - Top of Page

    • Provide documentation that will verify the student's High School Completion Status to the Arkansas State University – Mountain Home Office of Scholarships and Financial Aid.
    • Documentation to verify High School Completion Status may include:
      • A copy of the student's high school diploma.
      • A copy of the student's final official high school transcript that shows the date when the diploma was awarded.
      • A copy of the student's General Educational Development (GED) certificate or GED transcript.
      • An academic transcript that indicates the student successfully completed at least a two-year program that is acceptable for full credit toward a bachelor's degree.
      • If State law requires a home-schooled student to obtain a secondary school completion credential for home school (other than a high school diploma or its recognized equivalent), a copy of that credential.
      • If State law does not require a home-schooled student to obtain a secondary school completion credential for home school (other than a high school diploma or its recognized equivalent), a transcript or the equivalent, signed by the student's parent or guardian, that lists the secondary school courses the student completed and documents the successful completion of a secondary school education in a home school setting.

    Proof of Selective Service Registration - Top of Page

    • All males that have reached the age of 18 must be registered with Selective Service to be eligible for federal financial aid.
    • Provide proof of registration for Selective Service or provide documentation that the student is not required to register for Selective Service to the Arkansas State University – Mountain Home Office of Scholarships and Financial Aid.
    • Both registering for Selective Service and proof of registration with Selective Service can be accomplished online at http://www.sss.gov.

    Proof of US Citizenship - Top of Page

    • Documentation of Proof of U.S. Citizenship is needed. 
    • Please provide documentation to the Arkansas State University – Mountain Home Office of Scholarships and Financial Aid.

    Spouse's 2013 IRS Tax Return Transcript - Top of Page

    • A married student chosen for verification whose spouse has filed or will file a separate 2013 Federal Income Tax Return with the IRS MUST provide the Arkansas State University – Mountain Home Office of Scholarships and Financial Aid a 2013 IRS Tax Return Transcript for both the student and the spouse as part of the requested Verification documentation.  You must set up an account with the IRS and log in to be able to download IRS Tax Return Transcript(s).

    • The FAFSA was not signed by the student.  A student's signature is required on the Free Application for Federal Student Aid (FAFSA).

    • A student chosen for verification who filed or will file a 2013 Federal Income Tax Return and has not already used the IRS Data Retrieval Tool must use the IRS Data Retrieval Tool, if possible, or provide IRS Tax Return Transcript(s) as part of the requested Verification documentation.
    • If possible, log into the student's FAFSA at https://fafsa.ed.gov/ and complete the IRS Data Retrieval Tool in the Student's Financial Information.  Remember, for a dependent student, both the student and parent must sign the FAFSA with their Federal PINs each time a correction is submitted.

    Student has One or More Loans in an Active Bankruptcy Status - Top of Page

    • Provide a copy of all currently active and/or completed bankruptcy filings to the Arkansas State University – Mountain Home Office of Scholarships and Financial Aid. 
    • A student may remain eligible for Federal Financial Aid while in active bankruptcy proceedings.  However, Federal Loans that have been deferred due to bankruptcy proceedings must have been in good standing (Not in Default) prior to any deferment based on bankruptcy.

    Transcript Needed - Top of Page

    • An official academic transcript from a post-secondary educational institution previously attended must be received and interpreted by the Registrar/Admissions Office before Federal Financial Aid may be offered.
    • Contact the Arkansas State University – Mountain Home Registrar/Admissions Office or the Office of Scholarships and Financial Aid to discuss which post-secondary educational institution(s) transcript are not received.

    • Based upon data provided by the National Student Loan Data System (NSLDS), additional information may be required to determine your eligibility for Federal Student Aid. 
    • The Arkansas State University – Mountain Home Office of Scholarships and Financial Aid must review your records to determine what, if any, additional documentation or action is needed.

      • The student’s FAFSA indicates “Will File” was selected as the parent(s) tax filing status. This tax filing status must be changed before an offer of federal financial aid will be made.
