Welcome to Arkansas State University – Mountain Home! We are excited to have you as part of our team. To help you get started, we have provided you with this Toolkit for Workforce and Community Education Instructors. Please review this information carefully.

NEW INSTRUCTOR FORMS

Please complete the Instructor Forms below and return them to Peggy Spiegel at the Technical Center or scan and email them to pspiegel@asumh.edu. If you have questions about any of the forms, she will review them with you. Note: You will need Adobe Acrobat Reader to view these documents.

All community education instructors should print and complete the following forms and return them to Human Resources.

I understand the program has a specific course rotation that I am required to follow each semester.	* Initials * Date
I understand that all Funeral Science Education curriculum course work must be completed with a grade of “C” or better.	* Initials * Date
I understand that I must maintain a 2.5 GPA and will only be allowed to retake courses from which I withdrew or failed in the first semester upon admission to the program.	* Initials * Date
