Requested Documents


Submit requested documents to the Office of Scholarships and Financial Aid at ASU-Mountain Home:

  • DD214 Form – If veteran status on FAFSA doesn’t match VA records.

  • Driver’s License – To verify name, date of birth, and social security number.

  • INS Alien Registration – Provide Alien Registration from Immigration and Naturalization Services.

  • Marriage License – To verify marital status on FAFSA.

  • Military Orders – To confirm active duty status (excluding training).

  • Parent 1/2 Driver’s License – To resolve FAFSA mismatched information.

  • Parent 1/2 Social Security Card – To resolve FAFSA mismatched information.

  • Parent’s 2021 or 2022 W-2 – For verification if no tax return was filed.

  • Parent's Divorce Decree – To verify parental marital status on FAFSA.

  • Student’s 2021 or 2022 W-2 – For verification if no tax return was filed.

  • Student's Birth Certificate – To verify reported information.

  • Student's Divorce Decree – To verify marital status on FAFSA.

  • Student’s Social Security Card – To verify reported information.

  • Valid Government ID – Must be current (driver's license, state ID, or passport).

  • Parent’s 2021 or 2022 Tax Return – Provide IRS Tax Return Transcript.

  • Proof of U.S. Citizenship – Submit documentation as needed.

  • Student/Spouse Tax Return – Married students must provide separate transcripts if filing separately.

  • Loans in Bankruptcy – Federal aid eligibility may remain if loans were in good standing before deferment.

  • FAFSA Signature – Submit signed Federal Signature Page if missing.

  • Transcript Needed – Provide official transcript from previously attended institutions.

Financial Aid Notices

Aid Suspended – Appeal Required
You are ineligible for Federal Aid due to not meeting Satisfactory Academic Progress. Submit an appeal letter to the Office of Scholarships and Financial Aid. Supporting documentation is recommended but not always required. The committee may request documentation before considering your appeal.

Federal Aid Overpayment
You have an unresolved Federal Aid overpayment from a previous school.

  • Submit a letter from the school confirming you no longer owe an overpayment.

  • Check your status at studentaid.gov under your Dashboard.

Loan Defaults
You have one or more defaulted Federal Loans.

  • Provide a letter from your loan servicer confirming the default is resolved.

  • Check your loan status at studentaid.gov.

Exceeds Loan Limits
You have reached or exceeded the Federal Direct Subsidized and/or Unsubsidized Loan limits. Your aid is suspended.

  • Contact the Office of Scholarships and Financial Aid for assistance.

Additional Information Needed
Your aid is suspended pending additional information.

  • Respond to the letter or contact the Office of Scholarships and Financial Aid for clarification.

Loans Discharged Due to Disability
If you received a Total and Permanent Disability (TPD) Discharge, you may not be eligible for new loans unless specific conditions are met.

Marital Status Change
Your marital status correction conflicts with your original FAFSA submission date.

  • Only update marital status if it was incorrect when you filed or if advised by a Financial Aid Administrator.

  • Review and correct your FAFSA if needed.

Direct Loan Entrance Counseling
You were offered a Direct Stafford Loan but must complete Online Entrance Counseling at studentaid.gov to receive the loan.

Parent Signature on FAFSA
Your parent did not sign the FAFSA.

  • Both student and parent must sign using their own FSA IDs at studentaid.gov.

  • Allow 3–5 business days for updates to process.

Pell Lifetime Eligibility
You are near or have reached the limit for Federal Pell Grants. Your eligibility may be limited.

Unusual Enrollment History (UEH)
Additional information is needed to verify your eligibility for Federal Aid.

  • Contact the Office of Scholarships and Financial Aid for guidance.

Admission acceptance status made at application due date is contingent on final grades.
Important Disclosures about the Program and the Funeral Service Industry
I understand the program has a specific course rotation that I am required to follow each semester.
I understand that all Funeral Science Education curriculum course work must be completed with a grade of “C” or better.
I understand that I must maintain a 2.5 GPA and will only be allowed to retake courses from which I withdrew or failed in the first semester upon admission to the program.
I understand that I am responsible for any costs associated with the program, including field trips and any required visits to the Mountain Home campus.
I understand that it is my responsibility to know the licensing requirements for any state in which I intend to practice.
I understand that students who have previously been convicted of a crime may be restricted from certain clinical facilities and may be ineligible for some state licenses.
I understand that students who have a physical disability should contact the state in which he/she plans to seek licensure to determine if the disability will affect licensure.
I understand that a current driver’s license is required prior to enrolling in a clinical or practicum course.
I have read and understand the program dismissal policies contained within the Funeral Science Student Handbook.
