STEP 1: Official Transcript Request

We will need official transcripts from all colleges or universities where you received degrees. Please contact your schools and request the official transcripts be mailed to the attention of ASUMH Human Resources at 1600 South College St., Mountain Home, AR 72653.

STEP 2: New Employee Forms

You will complete most new hire forms online, but some will need to be printed and submitted to Human Resources. Follow this link to the forms that should be completed online:

  • Completed I-9. The U.S. Citizenship and Immigration Services (USCIS) requires employers or an authorized representative to verify the right of our employees to work in the U.S. For remote employees, this process is more challenging because of the requirement to physically inspect the presented documents that verify employability. Please allow ample time to complete the requirements of this form, and please follow the instructions exactly to ensure proper completion of the federally required form. You may access the complete I-9 Instructions here.

  • Print the I-9 Remote Hire Form and Instructions and complete the Employee Information Section of the Remote Hire Form. You may access the complete I-9 Instructions here if needed.

  • Please open and electronically complete Section I of the Form I-9. Please click the button "Click to Finish" to verify all required fields are completed. Then print the entire document.

  • Take the printed I-9, your required forms of documentation, and the Remote Hire Form and Instructions to your current employer's Human Resources office or a local college or university's Human Resources Office. If another Authorized Representative is needed, please contact ASUMH Human Resources at 870.508.6204 for additional instructions.

  • Once the Authorized Representative has verified your original documents and completed and signed Section 2, make color copies of your original documents to attach to the I-9. Please include all the associated documents, the completed and verified I-9, and color copies of presented documents in your mailed packet.

  • It’s highly recommended that before mailing your paperwork, you fax or scan a copy to the ASUMH Human Resources Office for review to ensure the documents are completed correctly. We will review and ensure accuracy before mailing.

STEP 3: Handbooks

Please download and retain the appropriate handbook and make certain you are familiar with it.

STEP 4: Other Information

Email and Account Access: Once your background check is complete, Human Resources will initiate a request with Computer Services to create your ASUMH email account and portal account access. Your immediate supervisor will initiate requests for department specific account access and permissions.


Training Courses. You will receive several emails from ASUMH Human Resources with the subject line "Online Training Assignment" These emails will contain instructions and a link to complete the training online. These courses must be completed before adjuncts can be cleared to teach.


ASUMH Alert. ASUMH's emergency notification system, called ASUMH Alert, will enable us to communicate with you quickly in the event of an emergency. The ASUMH Alert system will send messages to email and text message to your cell phone. Standard text messaging rates will apply. The ASUMH Alert System utilizes the portal to maintain contact information. You will be provided instructions on how to log in to the MyCampus portal and sign up for text alerts.

Admission acceptance status made at application due date is contingent on final grades.
Important Disclosures about the Program and the Funeral Service Industry
I understand the program has a specific course rotation that I am required to follow each semester.
I understand that all Funeral Science Education curriculum course work must be completed with a grade of “C” or better.
I understand that I must maintain a 2.5 GPA and will only be allowed to retake courses from which I withdrew or failed in the first semester upon admission to the program.
I understand that I am responsible for any costs associated with the program including field trips and Training Camp.
I understand that it is my responsibility to know the licensing requirements for any state in which I intend to practice.
I understand that students who have previously been convicted of a crime may be restricted from certain clinical facilities and may be ineligible for some state licenses.
I understand that students who have a physical disability should contact the state in which he/she plans to seek licensure to determine if the disability will affect licensure.
I understand that a current driver’s license is required prior to enrolling in a clinical or practicum course.
I have read and understand the program dismissal policies contained within the Funeral Science Student Handbook.
