Grade Change Form

Used to change a student's official grade on their transcript.

Course Incomplete Form

Notification to the Registrar of an Incomplete grade. Failure to complete the course within the subsequent term will result in a final grade of 'F'.

Request for Additional Hours

This process facilitates a credit hour overload request for students seeking to enroll in units beyond the standard term allowance.
