Steps to Apply for Aid

1. Apply for admission to ASUMH

2. Apply for a Federal Student Aid ID username and password for the student and parent, if dependent, at fsaid.ed.gov.

3. Complete the Free Application for Federal Student Aid (FAFSA) on-line at fafsa.gov using the Federal Student Aid ID (FSA ID) username and password for the student and parent, if dependent for the appropriate school year.

  • Click here to review an online video on how to fill out the FAFSA.

  • Use the Federal School Code 042544 when filing the FAFSA to have the results sent to ASUMH.

  • File the appropriate FAFSA.

  • 2018-2019 FAFSA is for Fall 2018, Spring 2019, and Summer 2019

  • 2019-2020 FAFSA is for Fall 2019, Spring 2020, and Summer 2020

4. After filing the FAFSA, there will be additional paperwork to complete for the ASUMH Office of Scholarships and Financial Aid.

  • Watch the email address you provided on the FAFSA and your ASUMH student email for additional information requests.

  • All students should complete the Financial Aid Data Form once a year after the FAFSA is completed to update Title IV Authorizations.

  • 2018-2019 Financial Aid Data and Title IV Authorization Form.

  • 2019-2020 Financial Aid Data and Title IV Authorization Form.

  • Additional Documents may be required if you were chosen for verification. Many students who apply for Federal Financial Aid through the FAFSA are selected by the Central Processor to be verified. If you are selected, you will be required to provide additional verification documentation which will be requested on an as needed basis.

5. If applying for summer, in addition to the all the previous steps, you will also need to fill out the Summer Financial Aid Application

6. Summer 2020 Financial Aid Application will be available in late spring 2020. Complete the ASUMH Scholarship Application by March 15th each year for the next school year.

7. Freshman Arkansas residents attending college for the first time can apply for the ASUMH Academic Distinction Scholarship.

8. Review and apply for individual scholarships. Information on many locally sponsored scholarships are available on the ASUMH Scholarships page.

When You Receive An Award Letter

When an award is received from the ASUMH Office of Scholarships and Financial Aid it will be emailed to the address provided on the FAFSA.

  1. Review the letter for further instructions.

  2. If you have been offered more loan funding than you desire, return a copy of the award with the desired loan amount indicated.

  3. If you are awarded a Direct Stafford Loan on your award letter, ensure the following items have been completed at studentloans.gov.

    • Direct Loan Entrance Counseling

    • Direct Loan Master Promissory Note

Step Up, Reach Back Intern Application

An important part of the Leadership Intern program is service to others. Reaching back after you have been through the program and helping a new member of the program is encouraged. Consider the list of ideas below and mark at least two things you would be willing to do.

Statement of Commitment
By signing this form, I agree to the following:
  1. I will enroll in at least 6 credit hours per semester.
  2. I will maintain at least a 2.0 cumulative GPA.
  3. I will attend orientation, retreat, by-monthly meetings with my mentor(s), and any other required activities on the attached required activities list.
  4. I will participate in planning and executing a leadership event.
  5. I will provide feedback to the mentors.

Admissions decisions made based on mid-term grades will be contingent upon final grades.
Important Disclosures about the Program and the Funeral Service Industry
I understand this program is full-time and requires that I follow a specific course rotation of 14-16 credit hours per semester.
I understand that all Funeral Science Education curriculum course work must be completed with a grade of “C” or better.
I understand that students must maintain a 2.5 GPA and will only be allowed to retake courses from which they withdraw or failed in the first semester upon admission to the program.
I understand that students are responsible for any costs associated with the program including field trips and Training Camp.
I understand that when I receive my results for each section of the National Board Examination, I must forward a copy of such results to the program coordinator as a condition of graduation.
I understand that it is my responsibility to know the licensing requirements for any state in which I intend to practice.
I understand that students who have previously been convicted of a crime may be restricted from certain clinical facilities and may be ineligible for some state licenses.
I understand that students who have a physical disability should contact the state in which he/she plans to seek licensure to determine if the disability will affect licensure.
I understand that a current driver’s license is required prior to enrolling in a clinical or practicum course.
I have read and understand the program dismissal policies contained within the Funeral Science Student Handbook.
